What documents are required for carrier setup?

Carriers need: (1) Active FMCSA Operating Authority, (2) Signed W-9 Form, (3) Certificate of Insurance showing minimum $1M Auto and $100K+ Cargo with United Freight Logistics Inc listed as certificate holder, and (4) Notice of Assignment if using a factoring service.

What equipment types do you broker?
